From my own experience I can say that the most reliable resource to predict your scores on the real exam is NBME. With the score of 490 you should get about 85 on the real exam. Kaplan simulated exam is much more difficult than the real exam, so the result of 67% on this test is not bad. If you want to get higher score on the real exam (for example above 90) I would recommend to study for another 3-4 weeks and then take the second NBME exam. Then you will know what you can expect on the real exam.
Good luck to you!

Hi,
The general rule is - add about 10 -15 to the percentage to ge the 2 digit score. So, if you are in high 60's, you probably would get aroud 80 in 2 digit score. Is that good enough for you? I would say, wait until you get 75% so that you would get high 80's to 90's. You may need another revision requiring a month or so. GL!
