A 28 year old lady develops pain and swelling

A 32 year old lady develops pain and swelling of MCP's, PIPs wrists, elbows and knees over three weeks. She works in a local school as a Kindergarden teacher. No family h/o of RA or other rheumatological dx. The most likely diagnosis would be
A.Rheumatoid Arthritis
B.Systemic Lupus Erythematosus
C.Post infectious arthritis
D.Hepatitis B
E.None of the above.

RA-rarely involves small joints[/quote] While the above case is most likely post infectious, the statement RA-rarely involves small joints is not accurate. RA principally affects the small joints of hands and feet especially mcp, pip in hands and metatarsal joints in feet. Infact, in rheumatology, it is a routine practice to take x-rays of hands and feet to see for erosions. (I worked in Rheumatology for an year and so take my word on it!)

I THINK IT IS POST-INFECTIOUS.
IS SLE LIKELY TO CAUSE SUCH AN ACUTE FLARE-UP?
i think it is SLE ? post inf ....unlikely to involve so many joints
RA-rarely involves small joints
