patient note

By Anonymous - Posted on 05 May 2007

hi,
i am finding it difficult to write a legible patient due to several reasons. first, my handwriting isnt good, i fall short of space when i write a detailed pt note including all the headings and all negative points. it appears so jammed that i myself find it annoying. i am a slow typer. so typing isnt an option,i feel. is it ok if i omit some headings like PSH,ROS (if its non contributory) and the negative points in FH and SH inorder to save time n space?
thank you prab n aajakal!
follow the pattern given by first aid for cs. do write all headings and underline them. more than reading everything they want to c if u can write the notes properly. good luck
Hi,
First, I think you are probably trying to write a lot. The pt. note is supposed to be very focussed and should not contain too many negatives. You should also use abbreviations. I think you should practice typing which only takes a few days and then use the computer to type PN. If you want to try it, use the patient note generator software at usmlesource.com below which has a timer that warns after 10 mins. http://www.usmlesource.com/guest/patient_encounters/case1.html NB: Most people do not know they can type faster than they write.
