sp in pain

hi,
if the sp is lying on the table with sever pain, how should the introduction be?
1) hi mr.... im dr... shake hands..nice to meet u
or
2) hi mr...im dr... im here to ask u a few qs n do a physical examn to see what i can do to help u( no hand shake)
Hi,
If somebody is in pain, I would not try to shake hands since that may cause some discomfort and more pain to the patient. I would say, Mr. Smith, my name is Dr. XYZ, I am here to talk to you and do a physical examination. I am sorry you are in pain. I would be better able to help relieve your pain once I complete my history and physical examination.
