Hi,
Rheum and Endo are average in terms of competitiveness. It is not that difficult to get these. But you need to do some electives preferably at programs you are interested and get to know the people there. Also, you need to do some reseach in these areas. Most fellowships are filled by people who the program knows one way or the other. So, do electives. Get good LOR's. Doing chief residency also helps. There are programs that do sponsor for h1 in fellowship. One of our classmates is doing rheum at drexel university o h1. PS is not that critical for fellowship. all the best.
