It depends. It is an additional plus but it is not a requirement for most programs. It is hard to find externships. What you need for LOR's depends on the individual program that offers externships. You can call around and see if any of the local hospitals offer them. The CV is generated by ERAS once you apply. There is no other single US format if you are applying outside of ERAS. You can look at the sample ERAS application available on the left side links here and try to come up with something similar. All the best.
