Ortho Residency 2010

I have completed my medical school i.e. MBBS in 2004 from a reputed institution in India. I have also completed my Orthopedic residency from the same intitution in 2008. I now wish to apply for a residency in Orthopedics for Match 2010.i.e. residency program starting in June 2010. 1) Should I apply apply for a transitional year or should I apply for a preliminary year? Or can I apply for both? 2) Should the transitional year necessarily be in a general surgery program, or can I have my transitional year in Internal Medicine? 3) Can someone please send me links for sample statements of purpose for transitional/preliminary year ?
